If necessary, you can activate or deactivate a patient form. If active, the patient form is available for all patients to complete online. If inactive, the patient form is not available to the patients.
To activate or deactivate a patient form
How to get there

From the Settings menu, under Location, select Patient Forms.
The Patient Forms page displays.
Note: If the correct location is not already selected, use the Location menu to select it.

On your location's Patient Forms page, select a patient form.
Note: Inactive forms are listed in gray text, and active forms are listed in black text.
The options for the form becomes available.

To activate a patient form, set the Active toggle to On. To deactivate a patient form, set the toggle to Off.

(Optional) From the Form Expiration drop-down list, select how long the form is valid for before it expires and requires the patient to resubmit the form.
Example: If you select 5 months, the form is valid for 5 months after a patient completes the form. After 5 months from the completion date, the form becomes invalid and the patient needs to complete the form again.

ClickTap Save.
