A patient note is a note containing information in relation to the patient, such as a detailed recording of a conversation between the patient and a user. You can add a patient note to a patient's record.
Note: You can also add a patient note as a notification message that displays on the bottom-right corner of a page.
To add a patient note

On the Patient menu, under General, select Patient Information.
The patient's Patient Information page displays.
Note: If the correct patient is not already selected, use the Patient Search field to access the patient's record.

ClickTap the Notes tab.

On the Patient menu, under General, select Overview.
The patient's Overview page displays.
Note: If the correct patient is not already selected, use the Patient Search field to access the patient's record.

Scroll to the Patient Notes widget.

On the patient's Patient Information page > Notes tab, clicktap Add Note.
Tip: You can also add patient notes from the patient's Overview page > Patient Notes widget > Add button.
The Add Notes window displays.

(Optional) ClickTap the Date field and select a different date.
Note: By default, the date is set to today's date.

In the Patient specific note field, enter any important information regarding the patient.
Example: A conversation you have had with the patient in person or electronically.
Note: The note can be up to 5000 characters long.

(Optional) ClickTap Insert Date to add today's date at your cursor location in the Patient specific note field.

(Optional) To make the note into a notification message, under Notification Pages on the right side of the window, complete one of the following:

To add the notification to all available areas, select the Add all pages check box.

To add the notification to only certain areas, select the necessary check boxes.
Tip: To select all pages within a category (General, Insurance, Financial, Clinical, or Schedule), select the category check box. To select an individual page within a category, expand the category and select the page check box.
Note: If necessary, you can deselect the check boxes to remove the notification from certain areas.

ClickTap Save.
The note displays on the Patient information page > Notes tab and in the patient's Overview page > Patient Notes widget.

Notes with the same date are grouped together.

By default, notes display in chronological order in descending order, with the oldest displaying at the top. If you sort the list, Ascend Academic maintains your latest sort preference and also applies it to the Patient Notes widget.

If a note is set as a notification, it displays the
icon.
