Patient Forms Overview

Ascend Academic provides an online portal where patients can complete questionnaires (such as demographic and insurance information forms) and consent forms (such as financial policy forms and treatment consent forms). You can decide which forms are sent to your patients. You cannot edit the questionnaires, but you can customize the consent forms for your office. Using the portal, patients can conveniently complete forms from home prior to their visits. When a patient completes a form, the system automatically saves it to the patient's Document Manager.

• Questionnaire Language - Patients whose records have Spanish, French, or Italian as their preferred language will see questionnaires in their respective language. All other patients will see questionnaires in English.

• Dental History for Minors - In the Dental History Questionnaire, answering Yes to the question “Is the patient a minor?” triggers a change to the set of questions about the patient’s dental history. These additional questions are commonly useful to a pediatric dental practice.
