Ascend Academic provides an online portal where patients can complete their forms prior to or during their visits. When you activate the patient forms feature, you can send an electronic request to the patient so that they can conveniently access and complete their questionnaire or consent forms online. When the patient completes the forms, you can easily access the completed forms. The system adds efficiency and professionalism to your practice - as well as maintaining digital records - when working with patient forms.
