Sending Online Forms to a Patient

Online patient forms provide a streamlined process for patients to update their medical history or provide you with their insurance information. The link to fill out forms electronically can be texted or emailed to a patient. If the patient arrives for an appointment and their forms haven't been filled out, they can be manually sent to the patient via email or text right then and there.

Additional Information

• Once a form is filled out, it immediately displays in the Patient Forms section of the patient Overview page.

• You can't request a patient form that's not due. If you need to request a patient form, you need to change its expiration settings. To learn more, read Changing the due dates of patient forms in the knowledge base.
