Online patient forms provide a streamlined process for patients to update their medical history or provide you with their insurance information. The link to fill out forms electronically can be texted or emailed to a patient. If the patient arrives for an appointment and their forms haven't been filled out, they can be manually sent to the patient via email or text right then and there.

Once a form is filled out, it immediately displays in the Patient Forms section of the patient Overview page.

You can't request a patient form that's not due. If you need to request a patient form, you need to change its expiration settings. To learn more, read Changing the due dates of patient forms in the knowledge base.
