You can edit an evaluation form. Changes may include any of the following:

Modifying the general fields, including Name and Evaluation Type

Activating or deactivating the form
-
Adding, editing, or removing general questions, procedure-specific questions, and headers
Once modified, the changes only apply to new evaluation requests. They do not apply to in-progress and completed evaluation requests.
Note: You can manage evaluation forms according to your security rights.
To edit an evaluation form
How to get to there

From the Settings menu, under Academic, clicktap Evaluation Forms.
The Evaluations Setup page displays with the Evaluation Forms tab selected.

On the Evaluation Setup page > Evaluation Forms tab, select a listed evaluation form on the left side of the page.

(Optional) To activate a form, clicktap the Active toggle to set to Yes. Skip the remaining steps.
Note: Forms must have at least one question or the Comment Assistant enabled to be activated. If you attempt to activate a form that does not have at least one of these, you will be prompted to add one.

Deactivate the form.
Note: You can only edit deactivated evaluation forms.

ClickTap the Active toggle to set to No.

When prompted with a Deactivate the form window, clicktap Yes.

(Optional) If you want to prevent the form from being used to complete a session or competency evaluation, skip the remaining steps and leave the Evaluation Forms tab.

Under General, edit the fields as necessary.

(Optional) Under Tooth chart, select checkbox Enable tooth chart to modify the settings as necessary.

Under General Questions, modify the headers and/or general questions as necessary.

To add a header, clicktap Add Heading and enter the title of the header.

To modify a header, select the header, enter a new title, and clicktap outside the field.

To remove a header, click the X icon.

To add a general question, complete the following:

ClickTap Add Question.

In the generated field, enter the question.

In the Grading Scheme field, enter a grading scheme title, and select from the drop-down list.

If the question is optional, deselect the Req* checkbox.

If the question is critical, select the Critical checkbox.

To modify a general question, complete any of the following:

Select or deselect the Req* checkbox.

Select or deselect the Critical checkbox.

In the Question field, modify the question.

In the Grading Scheme field, enter a new grading scheme title, and select from the drop-down list.

To remove a general question, clicktap the X icon.

Under Procedure Specific Questions, modify the questions as necessary.

To add a procedure-specific question, complete the following:

ClickTap Add Question.

In the generated field, enter the question.

In the Code Range fields, enter the procedure code range the question applies to.
Note: If the question applies to multiple procedure code ranges, clicktap the icon
and enter the procedure code range in the generated fields. -
In the Grading Scheme field, enter a grading scheme title, and select from the drop-down list.

If the question is optional, deselect the Req* checkbox.

If the question is critical, select the Critical checkbox.

To modify a procedure-specific question, complete any of the following:

Select or deselect the Req* checkbox.

Select or deselect the Critical checkbox.

In the Question field, modify the question.

In the Grading Scheme field, enter a new grading scheme title, and select it from the drop-down list.

To remove a procedure-specific question, clicktap the X icon.

(Optional) To view the form, clicktap Preview.

When ready, clicktap Save.
Note: If you have not activated the form, the Activate the form window displays. If you want to activate the form, clicktap Yes. If you want the form to stay inactive and out of circulation, clicktap No.
The modified evaluation form displays in the list.

(Optional) If necessary, print the form.
